L’épaule est un complexe de cinq articulations. Cela permet des mouvements très amples mais la rend aussi instable. Cette instabilité doit être compensée en permanence par l’activité des muscles alentours. Cela les rend sujets à la fatigue, aux blessures, et aux inflammations.

Le sport, les activités de la vie quotidienne ou les chutes réceptionnées sur la main contraignent la zone de l’épaule et engendrent des douleurs.

Les pathologies.

L’épaule est constituée de différents éléments osseux, tendineux, capsulaires, et ligamentaires. Chacune de ces structures est susceptible de s’inflammer ou de souffrir en cas de blocage ou de surmenage.

Les tendinites, pathologie des tendons

Les tendons sont les attaches du muscle à l'os. La coiffe des rotateurs est un ensemble de tendons régissant la plupart des mouvements de la gléno-humérale. Les tendinites de la coiffe sont fréquentes. Elles sont favorisées par certains sports utilisant le membre supérieur ou les travaux répétitifs, à la chaîne notamment.

Les capsulites, quand la capsule est touchée.

Ce sont les inflammations de la capsule articulaire. La plupart des articulations sont de type synovial, du liquide est présent dedans pour la lubrifier et nourrir le cartilage. La capsule enferme le liquide synovial pour qu’il ne s’échappe pas de l’articulation. Cette capsule peut s’inflammer et rapidement géner le mouvement. Cela peut se produire à la suite d’une longue immobilisation quand on a le bras en écharpe par exemple. Dans les cas les plus graves, on peut avoir une capsulite rétractile qui empêche tout mouvement. On parle aussi d’épaule gelée.

En l’absence de pathologie lésionnelle, la dysfonction est à l’origine de la douleur.

La dysfonction d’une seule des cinq articulations entraîne la réduction de l’amplitude des quatre autres et de l’épaule en général. Ainsi à chaque geste du bras, les mouvements sont plus durs à effectuer. Il peut parfois être impossible d’en faire certains.

De même, les mouvements du bras peuvent être limités par des dysfonctions de la colonne cervicale ou dorsale, du thorax ou du coude. Ces blocages créent des chaînes dysfonctionnelles et se répercutent sur l'épaule.

Enfin les dysfonctions viscérales ou du diaphragme peuvent manifester des douleurs d'épaule.

Douleurs d'épaule, de la clavicule, de l'omoplate ou à la base du cou

Difficulté à lever le bras

Douleurs dans les grandes amplitudes de mouvement (lors de la pratique d'un sport par exemple)

Lourdeur, pesanteur dans le bras

Névralgie cervico-brachiale d'origine non-discale, sensation électrique dans le bras

Que fait l'Ostéopathe ?

Après avoir exclu à travers une anamnèse et un examen clinique

approfondis les causes organiques pouvant engendrer une douleur d'épaule (ex: pathologies inflammatoires, NCB d'origine discale, pathologies du foie et de la VB)

L'ostéopathe va en premier lieu, grâce à des tests spécifiques, examiner l'origine de la douleur et déterminer le tissus atteint (musculaires, neurologiques, articulaires...)

Le traitement ostéopathique consistera à libérer, par des manipulation non brutales, les tensions responsables de la douleur d'épaule.
